Introduction
All of us who own or work with horses will encounter moments when a horse is reluctant, fearful, or resistant to handling and restraint. How we respond in those moments matters for the safety of the people involved, but also for the long-term wellbeing and future behavioural responses of that horse. Traditionally, equestrians have used physical force to compel compliance through discomfort or immobilisation. However, a growing body of research, grounded in equine learning theory and welfare science, is challenging these practices and providing an alternative, more ethical approach.
The Problem
Horses frequently resist or react negatively to handling for the vet or farrier, and this can often lead to safety problems for people and horses. A survey of veterinarians found that 95 per cent reported working with horses they considered difficult at least several times each month, and more than 80% (as many as one in four vets) had sustained at least one injury caused by the horse they were treating within the previous five years. Of a total of 579 injuries recorded among respondents, 15 per cent required hospital attendance and another 11 per cent needed a visit to a general practitioner, with 37% of injuries resulting in ongoing discomfort or loss of function. Trying to examine and treat a painful or stressed animal is a dangerous business.
From the surveys, the respondent responses showed that the most commonly encountered unwanted behaviours included horses being pushy or bargy, unwilling to stand still, and horses described as needle or head-shy. What we know is that many of these problem behaviours arise as a consequence of learning, emphasising how important it is to be able to understand how such behavioural responses develop and how to avoid or address them when they arise.
The ‘mechanics’ of learning
Horses form connections between events, actions, and outcomes and retain those associations over time. This capacity for rapid, durable learning is central to both the problem and the solution when it comes to restraint.
Classical Conditioning
Classical conditioning, famously associated with Pavlov and his dogs, occurs when a horse learns to associate a neutral stimulus with an outcome that already produces a natural response. A horse that is repeatedly forcibly restrained using a headcollar may quickly come to associate having the headcollar put on or the headcollar itself with fear and discomfort, becoming difficult to catch and handle long before any procedure begins. The headcollar has become a conditioned stimulus predicting an aversive experience.
Operant Conditioning
Operant conditioning involves learning as a result of the consequences of behaviour and learning occurs when the behavioural response is rewarded (reinforced) or punished. Learning occurs as due to:
• Positive reinforcement (R+): adding something pleasant to increase the likelihood of a behaviour being repeated (e.g., a food reward for standing still).
• Negative reinforcement (R-): removing something unpleasant when the desired behaviour occurs (e.g., releasing pressure when a horse yields). This is the primary mechanism underlying traditional horse training with aids and pressure.
• Positive punishment (P+): adding something aversive to reduce a behaviour (e.g., a sharp jerk on a chain lead rope).
• Negative punishment (P-): removing something pleasant to reduce a behaviour (e.g., turning away and withdrawing attention).
Well timed, proportionate and consistently applied negative reinforcement (or pressure release) can be effective and welfare-compatible when used correctly. Difficulties arise when timing of pressure release or food reward is poor, pressure is too great and/or reinforcement application is unpredictable. Punishment-heavy approaches will increase fear and confusion, impair learning, and can damage the horse-human relationship.
The Impact of Fear on Learning
Fear is not simply an unpleasant emotion, it actively interferes with the cognitive processes required for learning. A horse that is experiencing fear or high arousal is in a state governed by the amygdala and the stress response system. Cortisol rises, heart rate increases, and the horse enters a state oriented around survival rather than learning. In this state, the horse cannot effectively process new information or form calm, positive associations. This is why attempting to force a fearful horse through a procedure rarely teaches the horse that the procedure is safe; it simply adds another layer of aversive experience to the association already present.

How Difficult Horses Develop
Our study specifically sought to understand how equine veterinarians perceived the development of difficult behaviour in horses. A recurring theme was the concept of learned associations: horses that had had early aversive experiences with handling, veterinary procedures, or restraint had often developed persistent negative responses that were then generalised to similar situations.
This process is well understood in behavioural science. A single highly aversive event can be sufficient to create a lasting conditioned fear response, particularly in prey animals for whom rapid threat learning is adaptive. Repeated mild aversive experiences can have a cumulative effect that is equally damaging over time. Horses do not forget. A horse that learned at age two that the arrival of a stranger carrying equipment predicts discomfort may still respond with heightened vigilance, tension, and resistance at age fifteen.

Importantly, difficult behaviour is not a fixed trait. It reflects the horse's learning history and current emotional state. Horses described as dangerous, aggressive, or unmanageable have, in virtually all cases, developed those responses through a history of interactions they found aversive, unpredictable, or inescapable. Understanding this reframes the problem: the question is not how to suppress the behaviour, but how to change the underlying emotional association.
About the use of ‘Twitching’ for restraining horses
Twitching is among the most widely used forms of physical restraint in equine practice worldwide. Yet there is surprisingly little known about how it actually affects the horse, including its physiological and emotional impacts and how long any effects last.
Twitching involves applying pressure or twisting force to sensitive areas of the horse's body in order to achieve temporary compliance. The most common sites are the upper lip, the ear, and, less frequently, a fold of skin.

The Upper Lip (Nose) Twitch
The upper lip twitch involves applying a loop of rope or metal chain to the upper lip and twisting until pressure is maintained. Researchers (Flakoll, Ali, and Saab (2017), investigated the physiological effects of both lip and ear twitching using heart rate (HR), heart rate variability (HRV), and salivary cortisol as indicators of autonomic nervous system activity and stress.
Their findings for the lip twitch were nuanced. When applied for up to approximately five minutes, the lip twitch produced a significant decrease in heart rate and an increase in HRV, indicating a shift towards parasympathetic (calming) dominance. This is consistent with earlier work by Lagerweij et al. (1984), who proposed that the lip twitch acts similarly to acupuncture, stimulating mechanoreceptors in the highly innervated tissue of the upper lip and triggering endorphin release, thereby producing a mild analgesic and sedative effect.
However, this calming effect has important limits. When a lip twitch was maintained for longer than five minutes, heart rate increased and HRV fell, indicating a return to sympathetic dominance. The ‘calming’ window is therefore a narrow one. Furthermore, horses that are twitched repeatedly or for excessive durations may develop anticipatory fear responses to the process itself, undermining any short-term benefit.
More recent behavioural research has added further complexity. A 2025 study examining stress and pain indicators during lip twitching found increased head movements, reduced time spent in a natural head position, and unequal forelimb weight distribution during twitching intervals. These patterns are associated with discomfort and pain in other contexts, such as laminitis. The authors noted that the immobilisation response seen in some horses during twitching may reflect a passive coping style linked to heightened anxiety rather than genuine calm, particularly in horses with specific personality profiles.
In summary, the upper lip twitch, applied carefully, briefly, and as a last resort before a short procedure, may have a legitimate place in emergency equine care. However, it shouldn’t be viewed as a substitute for training, and its use must be moderated by attention to the horse's behavioural and physiological responses throughout.
The Ear Twitch
The ear twitch involves gripping and twisting the horse's ear, sometimes by hand and sometimes with a mechanical device. The findings of Flakoll et al. (2017) here are unambiguous and concerning. Ear twitching produced significant increases in heart rate and salivary cortisol levels, indicating activation of the stress response and a shift to sympathetic dominance. There was no evidence of any analgesic or calming mechanism comparable to that seen with the lip twitch.
The authors described the immobility produced by ear twitching as a fear- and pain-induced freezing response. A horse that stands still during ear twitching is not a relaxed or calm horse; it’s likely to be in a state of tonic immobility related to fear.
This has significant welfare implications. A horse immobilised through fear is experiencing genuine distress, and the experience is likely to consolidate a strong negative association with the handler, the environment, and any procedures being performed. Far from resolving difficult behaviour, ear twitching is likely to entrench it. A horse that has experienced repeated ear twitching may become severely head-shy, difficult to bridle, resistant to clipping around the head, and generally more fearful of handling. Given evidence of aversive effects and the availability of better alternatives, ear twitching is difficult to justify on either welfare or practical grounds.
Skin Twitching
Skin twitching, which involves grasping a fold of skin (typically on the neck) and twisting or pinching it, is less commonly discussed in the literature. The presumed mechanism involves stimulation of skin mechanoreceptors, but the evidence base is poor. What limited research exists on comparable techniques in other species suggests that such methods rely primarily on pain and distraction rather than any endorphin-mediated effect. In the absence of evidence for a welfare-compatible mechanism and with effective alternatives available, skin twitching cannot be recommended as a routine restraint method.
The Long-Term Significance of Aversive Restraint
Perhaps the most important argument against routine aversive restraint is not the immediate distress it causes, but the behavioural trajectory it causes. Horses that experience frequent aversive handling do not habituate nor learn to tolerate it. They sensitise. Each aversive experience adds to the cumulative negative association, broadens the range of stimuli that trigger a fear response, and increases the likelihood that the horse will respond more intensely the next time.
The Pearson, Waran et al. (2021) study found that difficult behaviours were reported significantly less frequently among veterinarians with more years of experience, which may partly reflect that experienced practitioners have developed skills that reduce conflict rather than escalate it. However, the study also highlighted that alongside chemical restraint, the equine veterinary profession tends to use physical restraint rather than behaviour-based approaches, and that there is little formal training in learning theory across the profession.
For horse owners and caretakers, who interact with their horses frequently, the cumulative impact of poor handling practices is even more consequential. A horse that encounters aversive handling during daily care, grooming, loading, clipping, or farriery work is receiving repeated lessons in the unpredictability and threat of human interaction. Over time, this creates a horse that is harder to manage, less safe to be around, and, crucially, more likely to injure itself or others during necessary procedures.
When Physical Restraint Might Be Necessary
It’s not the case that physical restraint may never be used in equine care. There are situations, including genuine emergencies, life-saving procedures with no time for training, or circumstances where a horse's welfare requires immediate intervention, where some degree of physical restraint may be the least-bad option available.
The key point is that restraint, when used, should be the exception rather than the rule, and should always be understood within the context of its long-term consequences for the horse's learning and emotional state.
Cooperative Care
An alternative is training the horse to be a willing participant. Cooperative care is defined as training animals using positive reinforcement to cooperate and actively participate in husbandry and veterinary procedures. The approach originated in zoo settings, where it’s been used successfully for decades with species ranging from elephants and big cats to large herbivores. It has now been extensively developed for companion animals and is starting to be applied in the equine world. The core principles are fairly straightforward, though they require consistency and patience to implement well.

Horse Agency
Central to cooperative care is the concept of agency: giving the animal meaningful control over what happens to it. In practice, this is implemented through what is called a start signal. The horse is trained, using positive reinforcement, to perform a specific action (such as touching a target with its nose, placing a foot on a stand, or maintaining a particular head position) that signals to the handler that the horse is ready to begin the procedure. The handler begins only when the horse offers this behaviour and stops immediately if the horse withdraws it.

This approach fundamentally changes the nature of the interaction. Rather than being an object to which things are done, the horse becomes an active participant with the ability to communicate consent. Research across species has shown that perceived control over aversive events significantly reduces their stressful impact. Even simple choices, such as which food to eat first or which direction to walk, have measurable positive effects on an animal's affective state and optimism. Cooperative care harnesses this well-established principle and applies it directly to handling and veterinary care.

Systematic Desensitisation and Counter-Conditioning
For horses that already have established fear responses to handling or veterinary procedures, the pathway to cooperative care begins with systematic desensitisation and counter-conditioning. Systematic desensitisation involves exposing the horse to a very mild version of the feared stimulus, below the threshold at which it provokes a fear response, and gradually increasing the intensity as the horse demonstrates calm tolerance at each stage. Counter-conditioning pairs this exposure with something genuinely positive, such as palatable food, to build a new, positive association with the previously feared stimulus.

Attempting to accelerate it by pushing the horse beyond its threshold of tolerance, even briefly, can set back progress significantly and risks re-traumatising the animal. However, when conducted at the horse's own pace, systematic desensitisation and counter-conditioning reliably and durably change the emotional associations that drive fear-based behaviour.
Positive Reinforcement in Equine Handling
The use of positive reinforcement (R+) in horse training has a strong evidence base. Positive reinforcement is effective for teaching horses a wide range of behaviours, from loading into a trailer to tolerating injections, and has been shown to improve the human-horse relationship, reduce fear responses, and increase the horse's willingness to approach and interact with humans. Rewards should be something the horse values, such as a scratch or food treat, or a secondary reinforcer such as a clicker.
A common concern among horse owners is that positive reinforcement using food will produce horses that are pushy or mouthy. This risk is real but manageable and is primarily a function of poor timing and implementation rather than the method itself. When positive reinforcement is applied systematically, with clear criteria for what earns a reward and consistent rules about when food is available, horses learn the association and the ‘rules’ and do not become demanding or dangerous.
Practical Applications for Cooperative Care
Cooperative care can be applied to virtually any procedure that horses regularly need to tolerate, including:
• Injections and blood sampling: horses can be trained to offer a specific body part and hold still using a target, with food rewards contingent on calm behaviour during and after the procedure.
• Clipping: systematic desensitisation to the sound and sensation of clippers, paired with high-value food rewards, can transform most horses that are reactive to clipping into horses that tolerate it willingly.
• Foot/leg handling and farriery: progressive work to associate foot lifting and holding with rewards, and to train the horse to maintain the position voluntarily, reduces both the risk of injury and the stress on all involved.
• Veterinary examinations: target training to allow examination of the mouth, eyes, and ears; desensitisation to stethoscopes, thermometers, and syringes; all are achievable with a structured training programme.
• Loading: arguably one of the most common sources of conflict between horses and humans, trailer loading is highly amenable to cooperative care approaches and has an excellent evidence base.

The investment of time in this approach pays substantial dividends. A horse that has been trained using cooperative care principles is arguably easier and safer to handle across all contexts, requires less chemical sedation for veterinary procedures, and is significantly less likely to cause injury to the people working with it.
Professional/Personal Responsibility
Much of the advice circulating in equestrian communities about managing difficult horses reflects outdated assumptions about dominance, hierarchy, and the need to establish control through physical means. This approach is not supported by the science of animal behaviour and likely to make problems worse rather than better. This creates a shared responsibility. Horse owners and caretakers are encouraged to seek out training in equine learning theory and cooperative care principles, to question advice that relies heavily on force or punishment, and to work collaboratively with veterinarians and behaviourists who understand and apply evidence-based methods. Veterinarians and equine professionals, in turn, have a responsibility to keep pace with the behavioural science literature and to communicate its findings clearly to the horse-owning public.
Horses are capable, sensitive learners whose behaviour is shaped by their experiences. Cooperative care, grounded in an understanding of equine learning theory and implemented through positive reinforcement, systematic desensitisation, and the provision of genuine agency, offers a more effective and more humane alternative. It requires investment of time and skill, but the evidence from multiple sectors of animal care confirms that it reliably produces horses that are easier to handle, safer to be around, and, in the truest sense, better cared for.
